The details are grimly straightforward. The developer apparently issued instructions that Claude Code interpreted as permission to perform destructive operations on production resources. The agent didn't pause to confirm. It didn't flag the irreversibility of what it was about to do. It executed — deleting the database, the snapshots, and with them, years of accumulated records. The literal compliance was, by any engineering standard, a total system failure disguised as a successful command execution.
